Decision evidence captures rationale and supporting facts. Later, people and agents retrieve an auditable answer—not a generated story.

Why this is needed

Auditors ask “why did we decide X?” and only find email threads.
AI without evidence invents plausible but false rationales.
Pairs with Decision Memory: rules + approvals + evidence.
